When possible, medications should be given to students at home. Medications may be administered during school hours, if necessary. Please download and complete the appropriate form below if your child will need medications at school.
Prescription Medications: Prescription medications require parent/guardian and doctor's written consent on the Rock Hill School District Medication Authorization Form. Return the completed form to the school nurse's office along with the medication.
Non-Prescription Medications: Over the counter medications such at Tylenol or Ibuprofen require parent/guardian written consent. The Non-Prescription Medication Administration Form should be completed and brought to school nurse with the medication to be stored in the nurse's office. Remember no student should carry any medication on them. As needed medications will be given according to the instructions on the packaging.
Field trips and medications: All medication (except nebulizer treatments) that have been authorized to give at school, will accompany the student on field trips. In the absence of the school nurse, medications will be transported and administered by a teacher or other authorized Rock Hill School District employee. AT NO TIME WILL MEDICATIONS BE TRANSPORTED OR ADMINISTERED TO STUDENTS BY CHAPERONES.
