In order for school nurses to give the best care during our students during the school day and be an informed resource for your family, teachers, and Student Intervention Team (SIT), we must have the most current information regarding each student's medical/health and mental conditions. At the beginning of each school year or for transfer students (upon admittance), parents/guardians are asked to complete a Student Health History Form (SHHF). The SHHF makes the nurse aware of any current medical conditions, medications, and authorization to give over-the-counter medication as listed on the bottom of the form. If you have completed a SHHF at a school within the Rock Hill School District, this form will be valid for the remainder of the year at Lesslie Elementary. I will contact you upon transfer to update the form. If you have not completed a SHHF this school year, please do so as soon as possible.
There are also instances when the nurse or Student Intervention Team (SIT) need additional information from your child's doctor in order to initiate services to maintain optimal health and safety and/or assist your child academically.
If you have been contacted by me, guidance counselor, school psychologist, or principal regarding additional information being needed, please consider completing a Medical Release of Information. Our goal at Lesslie Elementary School is to equipt all of our students with the tools they need to perform to their maximum academic potential.
